2025-2026 School Enrollment Form

To enroll a new student at NSMA: Complete this entire form Provide school with copies of: Official Birth Certificate, Immunization Record or Waiver. Student will not be officially enrolled until these documents are received.

2025-2026 Pre-K Enrollment Application

The following must be turned in to NSMA Office in order to be officially enrolled: Admission form with $50 admission fee (paid at the NSMA Office) Student's legal state birth certificate Certificate of immunizations (or waiver) from the Department of Community Health

AFFIRMATION OF PRIOR DISCIPLINE RECORD

This is a REQUIRED form for all students applying to enroll at North Star Montessori Academy (Grades K-12th). A willful false statement on this affirmation will result in a report to the appropriate authorities.
